Request A Prescription Transfer


Transfer a prescription to our pharmacy to take advantage of all the patient benefits we offer. The entire prescription transfer process takes 1-2 business days. Once transferred, compounding times can vary depending on a multitude of factors. If you would like to request a transfer, please contact us 7-10 days before you are out of medication or we can transfer it and keep the prescription on file at our pharmacy until you are ready to have it filled.  Email us at RPH@epiphanyaz.com with the following information to begin the transfer process and we will contact you once it is complete:

Request An Insurance Claim Form
We do not contract with insurance companies. However, if you would like to request an insurance claim form to be emailed to you, you can mail it to your insurance company to see if they will cover any portion of your compound. Email us at RPH@epiphanyaz.com
